If you’re applying for Social Security Disability benefits, get ready to fill out forms. One of the first forms you will need to fill out is the Disability Report form for adults: Form SSA-3368.
How Do I File Form SSA-3368?
Once you complete Form SSA-3368, you will need to file it with the Social Security Administration when you file for Social Security Disability or SSI benefits. If you’re applying for Social Security Disability, your Adult Disability Report (Form SSA-3368) is not part of your disability application.
If you file it online, you can collect your medical evidence and fill it out whenever you’re ready.
If you opt to apply in person, you can fill out the Adult Disability Report during your interview with the SSA claims representative.

What Information Do I Need to Complete Form SSA-3368?
When you fill out your Adult Disability Report, you will need to outline the nature of the disability for which you are applying for benefits. Your disability can be physical, mental or a combination of both. You will also need to provide information on the date of when your condition began, any medical treatment you have received and any doctors you’ve seen or medical facilities where you’ve been treated. You will also need to provide details regarding your symptoms and your medical history.
One of the sections that is most important is the section that asks how your disability prevents you from working and why you’re unable to perform your past jobs and any other type of job.
What Happens Once I’ve Filed Form SSA-3368?
This report form is the starting point for your entire Social Security Disability claim. Once you’ve completed it, an SSA claims representative will forward it to Disability Determination Services (DDS). DDS is the state agency that makes the initial decision on your disability case for the Social Security Administration.
At this point, your case is assigned to a disability claims examiner who evaluates your disability claim. It’s important that you provide as much information as possible so the examiner has a clear picture. Make sure all of your information is correct especially things like contact information for your doctors and medical treatment facilities. Double checking the information on your form will help your claim move through the system more quickly. If you forget things or put down incorrect information or leave things blank, your disability can be delayed for months.
